HEALTH, IMMUNIZATION & MEDICATION
Students must have a pass to visit the health room except in cases of emergency. The staff in the health room will sign them in to see to their needs. The child sill be given an information report, part of which will go home with the child that day for parents. The other portion may be kept for teacher documentation. Keeping parents informed as to the physical status of their child is very important to the staff of Arroyo Del Oso.

- Parents or guardians will be contacted immediately when a child needs to be sent home due to illness or injury.
- Work and home phone numbers MUST be current so contact can be readily made.
- An emergency number must be provided. This may be a neighbor, friend , relative, etc.
- Students who are ill are not permitted to walk home.
- If a student needs emergency care and a parent or guardian cannot be reached, the child will be transported to UNM Medical Center Emergency Room for medical attention.
- In accordance with the New Mexico State Law, the school must have a current immunization record on file.
- Each student must have an updated and signed Health Room Emergency card on file.
- If your child has a skin irritation or rash, APS policy dictates that s(he) needs a notification from a physician of it's identity and non-communicability.
